Participants under the age of 18 MUST have a parent sign it on their behalf. Participants over the age of 18 may sign it themselves. We MUST receive this waiver in writing (you may sign and email it in) PRIOR to you stepping foot on the ice. You must select “yes” in order to participate in the league.
